CPAP Alternatives: What Are Your Options If the Mask Isn’t Working for You

Frances Freeman September 28, 2026 3 minutes read

For many people diagnosed with obstructive sleep apnea (OSA), a CPAP machine is the first treatment recommended. It works by delivering a steady stream of pressurized air through a mask to keep the airway open during sleep. When used consistently, it can be highly effective. The problem is that many patients struggle to use it every night. Mask discomfort, dry mouth, noise, claustrophobia, and the hassle of traveling with equipment lead a large number of people to abandon therapy altogether. If that sounds familiar, the good news is that CPAP is not the only path to better, safer sleep.

    Effective Alternatives to CPAP Therapy

One of the most widely used alternatives is oral appliance therapy. A custom-made oral appliance, similar to a mouthguard or retainer, gently repositions the lower jaw forward during sleep. This helps keep the tongue and soft tissues from collapsing into the airway. Oral appliances are quiet, portable, and easy to wear, which is why many patients who could not tolerate CPAP find it much easier to stay consistent with this approach. They are often recommended for mild to moderate sleep apnea and for patients who have tried CPAP without success.

Another option focuses on the muscles themselves. Myofunctional therapy uses targeted exercises for the tongue, lips, and facial muscles to improve their tone and resting posture. Weak or poorly coordinated oral muscles can contribute to airway collapse, so strengthening them can reduce snoring and support easier breathing at night. This therapy is frequently combined with other treatments to improve long-term results.

For some patients, the root of the problem is structural. A narrow palate, a recessed jaw, or crowded teeth can limit the space available for air to move freely. Treatments such as Vivos orthotropics in Las Vegas and epigenetic orthodontics aim to encourage healthier development of the jaws and dental arches, creating more room for the tongue and airway. In cases where tooth alignment plays a role, clear aligner systems can also help expand the arches and improve overall oral function. These approaches look beyond symptom management and address the anatomy behind disordered breathing.

    The Takeaway

Struggling with CPAP does not mean you have failed at treating your sleep apnea; it simply means you may need a different approach. Leaving sleep apnea untreated raises the risk of high blood pressure, heart disease, daytime fatigue, and other serious health problems, so finding a solution you can actually live with matters. A dental sleep medicine provider can evaluate your airway, jaw structure, and sleep history to recommend the option that fits you best. If the mask is not working for you, it may be time to explore what else is possible.
